a) Top-up Date

b) Top-up Time

c) Top-up Method

d) Transaction No

II) Please send us a copy of your ID (either Identity Card, License, Passport, etc) showing your Full Name and Face.

BC给我回邮件说：The address on the ‘Confirmation of Payment Transaction’ does not tally with the Identity Card address.

难道要把身份证上的地址翻译成英文，再填写到这个表上吗？
